If you’re a new patient and would prefer to fill out some of the paperwork when it’s more convenient to you, click the links below to download the appropriate form. Print it, fill out the forms and deliver them to our New Patient Coordinator (at 20 South Market Street in Cortez) or to the clinic where your appointment is scheduled. Please note that all paperwork is due one week prior to your scheduled appointment, to ensure that we can become familiar with your healthcare needs by the time you arrive for your appointment.

Kameo Smith, DO grew up in Loveland, Colorado and attended medical school in Pomona California at the Western University of Health Sciences College of Osteopathic Medicine of the Pacific. She completed her residency in Greeley, Colorado. She directed a Community Health Center in Socurro, New Mexico. She has 2 dogs, loves to read, travel, scrap book, and watch the Broncos.
Smith said for many patients, prenatal care can feel routine, These appointments help doctors and nurses build stronger relationships with patients.“When people aren’t comfortable for whatever reason, they are more anxious,” Smith said. “So I think making a patient feel comfortable and at ease with something that is a very natural process is extremely important.”
Smith says when she counsels patients about what to expect in labor, she tells them to keep their options open. In her experience, things don’t always go as planned, but that doesn’t mean they don’t go well.
“It’s one of those things in life you have very little control over – and that is OK,” she said. “You have to be flexible, and respond to things as they change so you can have a safe outcome for everyone. Communication is a huge part of that.”

Marcelyn LePique, MD, OB/GYN Obstetrics and Gynecology

Dr. LePique is an obstetrician and gynecologist in Cortez, Colorado. She grew up in St. Louis, Missouri and went to college in St. Louis at Washington University on a full scholarship. She attended medical school at the University of Colorado, receiving her MD in 1986. She did her residency at St. Joseph Hospital in Milwaukee Wisconsin where she delivered LOTS of babies (over 10,000 to date)! Dr. LePique practiced for 10 years in Alaska, where she was Chair of the OB/Gyn Department, Chair of the OB Risk Management Committee, and also a Clinical Professor at the University of Washington School of Medicine. Dr. LePique is a Fellow of the American College of Obstetricians and Gynecologists, and has been Board Certified since 1993.
Dr. LePique loves pregnant women, and pregnant dogs. She has been a breeder of Golden Retrievers since 1980. When she is not delivering babies or puppies she likes to spend time with her husband, train and hike with her dogs, and swim. She is also an avid reader and a St. Louis Cardinals baseball fan.
Daniel Bohle, MD, OB/GYN

Dr. Bohle is a Board Certified OB/GYN specializing in gynecology. He attended medical school at the University of Iowa College of Medicine and completed his residency at the University of West Virginia Hospitals.
Dr. Bohle believes that having local experienced providers is what elevates the level of care in Cortez.
“I like to spend time with patients – reviewing complete history, presenting treatment options and answering all of their important questions.” -Dr. Bohle
Dr. Bohle enjoys traveling, especially to Italy, classical music, exploring national parks and reading.
