We provide primary care for the whole family. We also practice osteopathic manipulation, immunizations, well women’s care, pediatric medicine and more.

Print it and fill out the forms and deliver them to our New Patient Coordinator at 20 South Market Street in Cortez or to the clinic where your appointment is scheduled. Please note that all paperwork is due one week prior to your scheduled appointment.

Jeni Lovett, certified Nurse Practitioner
Jeni has been a nurse for over 10 years and has her Masters of Science in Nursing. She is originally from Montana and much of her family resides in Wyoming. She recently married a lifelong Cortez resident and plans to spend the rest of her life caring for the people of Montezuma County. Jeni, her husband, and family members are large supporters of healthcare and especially the Four Corners Child Advocacy Center . Jeni has been a flight nurse, ER nurse, house supervisor and through her experience has learned how much she loves whole patient care- finding the root of the problem and taking a holistic approach.
Jeni loves all things outside- horses, hunting, fishing, and camping. She has 2 blue heelers that she loves as much as her husband. Jeni has a special passion for pediatrics and women’s health as well.
