What is the Patient Portal?
The Patient Portal is a free, secure online tool that allows you to review contents of your Southwest Medical Group medical record and communicate with your provider(s). These services are available 24/7 online for your convenience.
What do I need to use the Portal?
You must be an established Southwest Medical Group patient and will need an email address, an internet connection and a PIN (Personal Identification Number) from your Southwest Medical Group office, which you will use only once to setup your account.
What if I don’t have an email address?
An email address is required to create an account on the Southwest Medical Group Patient Portal. If you do not have one, you may create an email account free of charge from various sources online. We have listed some email providers below:
How do I sign up?
Please visit your Southwest Medical Group office and a staff member will enroll you in the portal. At that time, you will receive a letter with a PIN. Once you have a PIN letter, please visit myhealth.swhealth.org and select Register. You will be prompted to accept our Patient Portal Terms & Conditions. Once accepted, you can create your user ID and password and setup security questions. Lastly, you will be asked to verify your identity and enter the PIN code. Once you have completed this process, you can throw away your PIN letter and begin using the Patient Portal with your username and password.
Please note, the information you enter when verifying your identity must match EXACTLY what we have in your record.
What information is available?
You can view information from your Southwest Medical Group Electronic Medical Record including some test results, medication and problem lists, allergies, upcoming and past appointments. You will also be able to send and receive secure messages from your physician’s office, as well as print, download or send your Chart Summary. Please note Southwest Memorial Hospital visit information will not be included in this portal.
Is there an age requirement to use the portal?
Yes, individuals must be a t least 18 y ears old to participate in the S outhwest Medical Group Patient Portal. Exception applies only to emancipated minors with supporting documentation.
Can I see my family members’ medical record information through the portal?
At this time we are not allowing patients to review their family members’ records via the Patient Portal.
What if I need more information than is available through the portal?
If you need a more complete record, please visit your Southwest Medical Group office and request a hard copy of your Medical Records.
What if I can’t remember my password?
On the home page for the Southwest Medical Group Patient Portal, there is an I forgot my password link in the Log-in box. Select that link, and the system will prompt you with 2 security questions (questions that were chosen by you when you verified your account). Type your answers and submit. Your password will be sent to the email address you provided when you setup your account.
