What is the Hospital Transformation Program?
The Hospital Transformation Program (HTP) is a quality-based initiative to improve the quality of hospital care provided to individuals enrolled in Health First Colorado (Colorado Medicaid).
HTP Goals
- Improve patient outcomes and experience through care redesign and integration of care.
- Improve the performance of the delivery system by ensuring appropriate care in appropriate settings.
- Lower Health First Colorado costs through reductions in avoidable hospital utilization and increased effectiveness and efficiency in care delivery.
- Accelerate hospitals’ organizational, operational and systems readiness for value-based payment.
- Increase collaboration between hospitals, their community health partners and other providers.
HTP is managed by the Colorado Healthcare Affordability and Sustainability Enterprise (CHASE), a government-owned business within the state’s Department of Health Care Policy and Financing (HCPF). The program will tie a hospitals’ Medicaid supplemental payments to its ability to meet specific patient-care quality goals, improve the health of priority patient populations, create meaningful community engagement, and reduce costs.

Measures Southwest Health System Identified for HTP
Southwest Health System’s (SHS) mission is to provide the highest quality healthcare to our community by bringing excellence, value, and service together to promote, improve and restore health.
- Follow up appointment with a clinician made prior to discharge and notification to the Regional Accountable Entities (RAE) within one business day
- Social Needs Screening and Notification
- Screening and Referral for Perinatal and Post-Partum Depression and Anxiety and Notification of Positive Screens to the RAE
- Collaboratively develop and implement a mutually agreed upon discharge planning and notification process with the appropriate RAE’s for eligible patients with a diagnosis of mental illness or substance use disorder (SUD) discharged from the hospital or ED
- Using Alternatives to Opioids (ALTO’s) in Hospital Emergency Departments (ED):
- Decrease use of opioids
- Increase use of ALTO’s
- Increase the successful transmission of a summary of care record to a patient’s primary care physician (PCP) or other healthcare professional within one business day of discharge from an inpatient facility to home

SHS will continue to partner with the community to meet the HTP goals by the following:
- Improve care coordination of behavioral health patients by enhanced partnerships and communications
- Collaborate with community partners to improve care coordination—from the inpatient to discharge setting—through enhanced discharge planning and referrals to community resources
- Identify social determinants of health to assist in the patient’s needs
- Continue treating opioid dependence in our emergency department, primary care, and inpatient settings
- Create a robust data reporting system to identify and address the specific needs of our Medicaid population
