Welcome to Southwest Health System. If you’re a new patient and would prefer to fill out some of the paperwork when it’s more convenient to you, click the links below to download the appropriate form. Print it, fill out the forms and deliver them to our New Patient Coordinator (at 20 South Market Street in Cortez) or to the clinic where your appointment is scheduled. Please note that all paperwork is due one week prior to your scheduled appointment, to ensure that we can become familiar with your healthcare needs by the time you arrive for your appointment.

Crysta joins us from Midland Texas where she practiced as a Physician Assistant for the past 5 years. Crysta received her PA-C from Texas Tech University Health Sciences Center, School of Allied Health Physician Assistant program and has a Master of Science in Physician Assistant Studies. She completed her undergraduate studies at Wayland Baptist University with a BS in Biology and Chemistry.

Physician assistants (PAs) are medical providers who are licensed to diagnose and treat illness and disease and prescribe medication for patients. ​

She has a special interest in diabetes and has experience in a wide array of procedures. Crysta has a strong rapport with patients, focuses on building relationships, and emphasizes individualized, personal care.

Dr. Daney is board certified in Family Medicine and treats families, pediatric patients and new babies. She attended Albany Medical college in Albany, NY 1998-2002, and served a residency at St. Mary’s Family Practice Residency in Grand Junction, CO.

I love taking care of entire families from newborns to grandparents and I love watching my patients grow up! I am interested in preventative care.
I live in Durango with my two kids and several horses/donkeys/dogs.  I enjoy the outdoors, riding my horse in the mountains, skiing and hiking. I’m also a “hockey mom”.

Appropriately diagnose and treat a variety of diseases and injuries in a general practice setting. Regularly evaluate infants and children to assess growth and development compared to average childhood patterns. Treats pediatric patients with minor illnesses, acute and chronic health problems and growth and development concerns.

Regularly refers patients to medical specialists or other practitioners for more specialized courses of treatment. Administers and prescribes appropriate courses of treatment, including pharmaceutical medications. Provides prenatal care to pregnant women, delivered babies and provided postnatal care to mother and infants.

She practices at Southwest Memorial Physicians Medicine and Southwest Memorial Hospital Family Birthing Center, where she treats people of all ages, including pediatric patients. Shawna M. Frost FNP-BC graduated from Graceland University out of Independence, MO as a family nurse practitioner. She has been practicing in Farmington, NM for the last 6 years with a sub-specialty in pediatrics and internal medicine. Established a sound knowledge base in the clinic and hospital setting with the encouragement of the practice physicians. Active member of the American Academy of Pediatric and American Academy of Nurse Practitioners. Mrs. Frost enjoys horseback riding, gardening, jeeping, and spending plenty of time with her husband and 2 teenage boys.  She is a proud member of the rural area of Cortez, Co, and is now accepting new patients at Southwest Memorial Physicians 970-565-4291.
